Photo Radar Form Downloads
We offer a number of forms available for you to use to handle your photo radar case. These forms are completely free. The forms are in various formats, depending on the form: Fillable pdf where you can fill out the form and print it, pdf format and Microsoft Word format. Motion to Set Aside Default
Use this form if the court has entered a default judgment, may have suspended your license as a result. This form is to ask the court to undo the judgment so you can get your license back and allow you to handle your case without being in default. This is a fillable pdf file so you can simply fill in your information and print it.

Motion to Strike
Use this form if the process server left papers at your door and then filed an affidavit saying you were served. Designates the Motion with a Limited Appearance therefor preserving your right to argue service and jurisdiction issues. Includes the Motion to Strike the affidavit and brief instructions.

Process Server Complaint Form
If you were improperly served and/or a process server said he/she served you when he/she did not, file a complaint using the Supreme Court's own form. The complaint gets filed with the Superior Court. This is the way to stop abuse of process by servers who leave papers on the ground and claimed they served you!
